    I just don't understand what happened here?

    Was it plugged into a charger? Who has an outlet that close to the tub? Shouldn't a GFCI prevent things like this?

    Yes, it was plugged in. Might not have had a GFCI.

    The charging cord for a cell phone carries 5.5v dc. Not nearly enough to burn anyone's hand.

    Cheap chargers can have dubious circuitry.
