If it was a weekly trip, then redundancy can be trumped by complacency every time.

Or, the odds simply caught up with them. It's a proven fact that given enough opportunities, ships will hit stuff.

It looks pretty bad when you run aground next to a lighthouse on a clear night with calm seas.
